We authorize access to your personal information by our employees and business associates only to the extent necessary to conduct our business of serving you. JotForm: Ensuring the privacy and security of your data is a top priority for us. We take precautions to provide an online form service with high grade security. The forms are HIPAA-compliant, which also include a business associate agreement (BAA) to ensure health information safely and securely. Furthermore, all the forms are served across protected 256 bit SSL (Secure Socket Layer) connection that uses a SHA256 Certificate. JotForm is compliant with the California Consumer Privacy Act (CCPA), which protects California residents by requiring businesses to handle their online and offline data in a responsible manner. The forms are fully compliant with the European Union’s General Data Protection Regulation (GDPR), the data protection law taking effect on May 25, 2018. As a condition of the user completing the online form, you agree to JotFrom Privacy Policy. If you do not agree, we provide an alternative option, for the user to download the word doc from the Site and email the word doc directly to if this is your preferred method. The emails provided and referenced are encrypted, to ensure the security of information.
